Most common reason for getting this with D-Link DWL-650 seems to be mismatch in voltage configuration. If the card CIS does not include a proper entry, you can instruct the driver to ignore Vcc differences by setting module parameter ignore_cis_vcc=1 (either in /etc/pcmcia/hostap_cs.conf or, for testing, by manually loading the module with 'modprobe hostap_cs ignore_cis_vcc=1' before inserting the card).
